Graduate School Application Forms

The following forms are in pdf format for admission and other graduate procedures. They are formatted to allow students to type their information.  You will need the free Adobe Acrobat Reader software on your computer to view these files.  If you are using the United States Postal Service, please mail materials to:

  • Recommendation Letter                                                       
    Two recommendations are required and should be submitted electronically by the reference to , or mailed through the United States Postal Service, or hand-delivered in a sealed envelope.  Access the suggested guidelines for writing a letter of recommendation. 

  • Statement of Purpose Form (Non-Degree Applicants Only)   
    Applicant should use this form to explain professional intent for seeking enrollment.


  • Checklist for Graduation Candidacy                                    
    This form is used by the student to keep track of documents required for graduation. Students are emailed an office copy from the Office of Graduate Studies following processing of the Application for Degree. The checklist informs students of documents missing to clear them for graduation.
  • Application for Graduate Degree                                         
    This form is submitted according to published dates each semester and must bear the student's signature, advisor's signature and signature of the department chair before submission to the Office of Graduate Studies. Form must be typed.

  • Request Replacement Diploma   This link will point you to the Office of Student Records to complete the Customer Service Intake Form to make your request for a replacement diploma.                                          


  • Application for Graduate Assistantship                                 
    Applicants must have GPA of 3.0 or higher to apply. Form must be typed.
  • Graduate Assistants Time Sheet  


  • Application to Enroll in 500 Level Graduate Courses                                
    Restricted to seniors in their last 12 hours of study. Limited to a maximum of hours approved by the Vice Provost for Academic Affairs and Graduate Studies. Form must be typed.

  • Substitution Request Form                                                                          
    This form must be approved by the appropriate department faculty and submitted to replace courses substituted in the student's curriculum. Form must be typed.
  • First Master's to Second Master's Course Approval Form
    This form is used to transfer courses from the First Master's Degree to a Second Master's Degree. This process is governed by the Second Master's Degree Policy that limits the number of course hours that can be transferred as well as restrictions on the six year time limit for coursework. Form must be typed.
  • Transfer of Credits
    Approved limit of up to six hours by advisor. Form must include official transcript documenting courses to be transferred. No courses accepted below grade of 'B.' Form must be typed.
  • Request to Review Student Record                                                             
    Request submitted by student to review admission file. ID must be presented with request. Form must be typed.
  • FERPA Student Release of Information Form                                             
    Authorization by student to release credential information to designee to conduct admission inquiry in absentia. ID must be presented with request. Form must be typed.
  • Request for Release of Student Information
    Student submits this form to request a copy of admission or graduation credentials from student file. Student must present ID with request. Form must be typed.
  • Appeal Form                                                                                                     
    Submitted by student for reconsideration of academic status. Form must be typed and emailed to .
  • Remission of Fee                                                                                           
    This form must be typed and submitted in advance for approval before the last day of registration each term. Student completes student information in sections I and II, and attaches copy ofStudent Detailed Schedule. Form is submitted to Graduate Studies for approval before submitting to Business Office for processing.
