Alcorn State University offers worker’s compensation coverage to all faculty and staff who have sustained an injury while performing his or her job on and off campus.
Any employee who sustains a minor injury should first report the injury to his/her direct supervisor/department head. In the case where a supervisor is not present, please report the injury to human resources or the university’s health facility.
All minor injuries should be handled by the University’s Health Facility. The Health Facility will complete a First Report of Injury Form and submit it to the Human Resources Department. However, an employee who sustains a serious injury, requiring major medical attention, should use the following forms:
For completion by the supervisor/department head: Supervisors workplace injury investigation report
For completion by the employee: Notice of Physicians Choice and Medical Authorization
