ORIENTATION INSTRUCTIONS
Welcome and Congratulations! We are delighted to hear that you will be joining us. Your role is vital in fulfilling the mission of our great university, and we know that this is the beginning of a mutually beneficial association.
As with anyone starting a new job, I’m sure that you have some questions. Please complete, print and sign the online employment forms and bring them to Human Resources when you attend your scheduled orientation session. If all forms are not completed during the orientation session, this may cause a delay in processing your payroll check. In addition, you must bring a picture ID and another employment document (i.e.; social security card, birth certificate) to verify your work eligibility status (this is mandatory).
If your new position (i.e. faculty, coaches, librarians, and highly compensated administrators) requires enrollment in either the State Retirement Plan (PERS) or the Optional Retirement Plan (ORP), you have thirty (30) days to make a decision. If a selection is not made within thirty (30) days, you will be automatically placed in the State Retirement Plan (PERS). All other full-time positions are required to enroll in the Public Employees’ Retirement Plan (PERS). When you report to the Department of Human Resources for your orientation session, an HR representative will provide you with ORP Retirement packets (if applicable). The necessary forms for enrolling in the Optional Retirement Plans (ORP) will be enclosed in that packet. Please read the retirement information in your new hire packet to make an informed decision.
In addition, the following information must be completed on selected insurance forms:
- Please complete all applicable sections on the front page of the Blue Cross/Blue Shield State Health Insurance form, and Section D on the back side of the form. (After completing Section D, this will validate whether you are a legacy or a horizon employee.)
- You should read, complete, and sign all documents before submitting to the Human Resources Department.
- When reporting to HR to submit employment forms and documents, please be sure to sign your Personnel Action Form (PAF) before leaving the office.
- The state retirement plans are mandatory; all other insurances are optional and may be waived.
You should bring completed forms to the Human Resources Department located in the Walter Washington Administration Classroom Building, Room 107 on your scheduled orientation date. Your official date of employment will be your scheduled orientation date, unless otherwise indicated on the Personnel Action Form (PAF) provided by the hiring manager.

Please bring all printed and signed forms with you to orientation. NOTE: ASU New Employee Orientation is held on the 1st-15th (Monday-Thursday) of each month.
