Vaccine Card Info Form

Getting your COVID-19 vaccine is an important part of a safe return to campus and campus activities this fall. We strongly encourage you to get vaccinated now so you can be fully vaccinated before school begins. If you have not been vaccinated, you can find available vaccine locations by visiting vaccines.gov.

If you are fully vaccinated, please submit your COVID-19 vaccine information and copy of your vaccination card/record by clicking the purple button at the bottom of this page. Students who have submitted validated COVID-19 vaccination information will not be required to quarantine or isolate if they are exposed to someone who has COVID-19 unless symptomatic. This will enable the students who have been vaccinated to continue attending in-person classes, to continue living on-campus, and to continue engaging in all campus activities. Students who have no ID been vaccinated will be required to quarantine and/or isolate, typically for 10-14 days, if exposed or test positive for COVID-19.


Submit Your COVID Test

Students that have recently gotten tested for COVID are asked to submit their test results online before coming to campus for the Fall 2021 semester. This isn't to be confused with the COVID vaccine card. Take a look below for an example of each, and if you are looking to submit COVID test results, click the gold button below:

What's the difference between a vaccine card and test results?

  • The COVID-19 Vaccination card is a small, paper card that has the same format, as seen below, wtih the CDC logo in the upper-right corner, and it contains information about which vaccine you have taken when you got your dose(s) of the vaccine.
  • A COVID-19 test result comes in many different forms, but is typically the size of a full letter-sized sheet of paper and will tell you somewhere on the page(s) whether or not you have COVID-19 (typically using the words positive/negative or detected/not detected) at the time of that test.
